Psychiatric examinations in official institutions of the Brazilian government include examinations of individual convicts – some of whom are highly dangerous – carried out by court decision. These individuals are taken handcuffed under police escort from penitentiaries to the examination site. In most Brazilian states, medical examiners or experts adopt the basic procedure of asking the police officers to remove the handcuffs from the convict for the examination to be carried out. This article analyzes, from the bioethical standpoint, the behavior (...) of 49 forensic psychiatrists in eight Brazilian states in relation to procedures adopted as protection measures against any possible danger posed by their examinees. In the context of a developing country, where sophisticated protection means are not available, the practice of keeping convicts handcuffed during such examinations is compared to other measures and more advisable alternatives are discussed from the ethical standpoint. Considering the dilemma between showing minimum respect for the autonomy of examinees and ensuring the security of examiners, the need to adopt equitable procedures based on the recognition of the different needs of examiners and examinees is addressed. ABSTRACT The bioethics of the so‐called ‘peripheral countries’ must preferably be concerned with persistent situations, that is, with those problems that are still happening, but should not happen anymore in the 21st century. Resulting conflicts cannot be exclusively analysed based on ethical (or bioethical) theories derived from ‘central countries.’ The authors warn of the growing lack of political analysis of moral conflicts and of human indignation. The indiscriminate utilisation of the bioethics justification as a neutral methodological tool softens and even (...) cancels out the seriousness of several problems, even those that might result in the most profound social distortions. The current study takes as a theoretical reference the fact that natural resources (which all of us are) are finite, and that corporeal, pleasurable and painful matters (which affect us all) are relevant. Based on these premises, and on the concept that equity means ‘treating unevenly the unequal’, a proposal of a hard bioethics (or intervention bioethics) is introduced, in defence of the historical interests and rights of economically and socially excluded populations that are separated from the international developmental process. The teaching of global ethics can be developed from several points of view. The goals of this education should be to seek respect for plurality and construct a fairer, more equal and more supportive world. The reflections put forward in the present chapter are not only based on the theoretical foundations of ethics and bioethics, but also especially on the geopolitical locus of where the authors live and work, which is the southern hemisphere. The objective of using this artifice was (...) to prevent local negative effects that an intendedly globalized project for teaching ethics might cause. The aim was also to indicate the conceptual bases and practices from which legitimate goals can be defined for this initiative, which can be applied for use in different parts of the world without unilaterality, interpositions, impositions or manipulations. Because of the difficulty in defining goals for teaching global ethics, the present analysis took the reference point of two real themes of the contemporary world: moral imperialism and the point of view of coloniality. Overcoming the distortions and inequities of moral imperialism that are now perceived in the relationships established among different individuals, cultures, countries and regions of the world, combined with coping with situations of coloniality – whether occasional or global – is part of a macro-ethical context that poses an obstacle to construction of non-discriminatory, equal and inclusive global ethics. Considering the barriers against construction of consensuses in this field, the present text proposes the use of education in global ethics as a tool that is applied for opposing situations of moral imperialism and coloniality. Finally, instead of proposing abstract goals that are set aprioristically, this chapter indicates the paradigm of corporeity as a universal marker for what should be sought morally and what should be avoided in education programs regarding global ethics. Thus, this chapter provides a minimal axiological basis upon which it will be possible to establish goals, thereby avoiding educational practices in the field of ethics that will become yet another factual element serving the maintenance of unilateral and hegemonic interests and an unjust global order. The text examines the article 11 of the Universal Declaration on Bioethics and Human Rights of UNESCO that deals with the principle of non-discrimination and non-stigmatization. Both concepts are related to the theme of human dignity, while discrimination is an inherent part of stigma: stigma does not exist if there is no discrimination. In this context, this paper aims to study the relationship between stigma, discrimination and HIV / AIDS. The study argues that to loosen the bonds that hold the (...) subjects that are attached to them is necessary questioning the broader forces - social, cultural, political and economic - that structure stigma, stigmatization and discrimination as social processes directly linked to production and reproduction of structural inequalities. It has been hypothesised that the reimbursement system pertaining to radiotherapy is influencing prescription practices for patients with cancer with bone metastases. In this paper, we present and discuss the results of an empirical study that was undertaken on patient records, referred to radiotherapy for the treatment of bone metastases, in a medium-size city, in southern Brazil, during the period of March 2006 to March 2014. Our findings seem to confirm this hypothesis: after a change in the reimbursement method, radiation (...) prescriptions were adapted accordingly, in order to maximise profits. Once such patients become highly vulnerable due to their diagnoses, they also become susceptible to a subtle form of exploitation; physicians let patients believe that more radiation will be better for their health, and they do so despite knowing otherwise, and as it seems, out of pecuniary interests. In this article the authors briefly sketch the nature of Brazilian bioethics. Bioethics emerged in Brazil later than in other Western countries and the 1990’s were the most important period for the spread of the discipline in the country. It is in this period that some structural elements of bioethics were established, such as research groups, regulation of Local Research Ethics Committees (Comitês Locais de Ética em Pesquisa – CEP), the creation of the National Commission of Ethics in Research with (...) Human Beings (Comissão Nacional de Ética em Pesquisa com Seres Humanos – CONEP) and the Brazilian Bioethics Society (Sociedade Brasileira de Bioética – SBB). With regard to theoretical work, Brazilian bioethics is clearly an importer of theories from countries central to the studies of bioethics, or, in another words, countries where bioethics first emerged and was established. The text deals with the concept of representation in its relation to the concept and sovereignty, in Thomas Hobbes (1588-1679). The work studied is Leviathan, of 1651. The research presents that sovereignty is founded through the original legal act, which establishes an artificial person to represent everyone. This artificial thing is the State. The political society, however, is different from its government, although they are functionally the same.

Background:Promoting patients’ rights is essential for defining the standards of clinical services within a country. Given their responsibilities, nurses can be the primary target for research to investigate the issue of patients’ rights within a healthcare system. As such, assessing the knowledge of nurses about patients’ rights is an essential step toward improving the quality of healthcare in limited resource settings like Sudan.Objectives:We aimed to assess the level of knowledge about patients’ rights among the nursing staff at Friendship Teaching Hospital (...) in Sudan.Methods:This hospital-based cross-sectional study was carried out at the Friendship Teaching Hospital in Sudan. We surveyed the totality of nurses (95) at the hospital using an amended survey. The data were analyzed in SPSS software using descriptive and inferential statistics.Ethical consideration:The study was approved by the Research Unit of Khartoum Ministry of Health and the Hospital administration. All respondents gave verbal consent prior to participating in the study.Results:Only 48.4% of the participants knew about the existence of the Sudanese Charter of patients’ rights. Nonetheless, our analysis found that 65.8% of nurses had acceptable level of knowledge (scored more than 75% of the total knowledge score) of patients’ rights, and none of the participant scored less than 50% of the total knowledge score. Finally, we found no statistical association between the knowledge score and demographic data, educational level, whether the participant knows about the existence of the Charter or not and a number of other factors.Conclusion:Nurses’ lack of knowledge about the existence of the Sudanese Charter of patients’ rights adopted in 2009 rights confirms the need for further efforts by Ministry of Health to promote the document. Furthermore, further research is needed to investigate the disconnect between nurses’ lack of knowledge about the existence of the charter and their awareness of the rights within the charter as well as the actual implementation of rights. Contributing to the efforts to foster business postgraduate students development toward becoming responsible business leaders is the goal of this study by examining the state of responsible management education in business postgraduate programs in Sudan. We examined perceptions and attitudes toward responsible management and its education among postgraduate-level students and constructed a comprehensive framework appropriate for developing responsible management education programs in under-developed countries. This study’s data were gathered via a structured questionnaire answered by 106 postgraduate business students from the (...) largest four Sudanese business schools and facilities. The students were distributed among the management, finance, and marketing fields. The students exhibited affirmative corporate social responsibility perceptions and attitudes with a heightened focus on acting ethically and a diminished focus on financial considerations. A key lesson learned from this study is the need for the continued development of CSR and general ethics across business school curriculums in Sudan. (shrink)

Before 1972 Somalia had no official writing system for its language. In spite of this, those who bred animals (camels, cattle, sheep, and goats) and who, owing to a lack of water in the country were forced to become nomads, had an authentic oral tradition that found its voice in a rich oral literature. This was well and truly oral in that it was composed, memorized, and passed on without having to resort to any type of writing or other means (...) of communication or recording. Migration is a global phenomenon growing in scope, and it can be associated with negative emotions such as sense of impending loss, fear of the unknown, and anxiety about those left at home. The objective of this exploratory study was to examine psychological distress and homesickness among Sudanese migrants in the United Arab Emirates. Participants were 1444 Sudanese migrants. The Second Version of the Dundee Relocation Inventory was used to assess homesickness, and the 28-item General Health Questionnaire was used to (...) measure psychological distress, depressive and anxiety symptoms, somatic symptoms, and social dysfunction. The results showed that older age and longer duration of residence in the UAE were associated with lower levels of homesickness, psychological distress, and depressive and anxiety symptoms. Further, homesickness was associated with higher psychological distress, somatic symptoms, and depressive and anxiety symptoms. Women and unemployed migrants had higher psychological distress, somatic symptoms, and depressive and anxiety symptoms compared with men and those employed, respectively. Being unmarried was associated with higher levels of depressive symptoms and homesickness, while those married or divorced/widowed showed lower levels of depressive symptoms and homesickness. It was concluded that there is a need to tackle unemployment among migrants in the UAE and address family reunion issues. (shrink)

The acceleration of the market globalisation process over the last three decades has internationalised clinical research and influenced both the way in which it is funded and the development and application of research practices. In addition, in recent years international multicentre randomised clinical trials have become the model par excellence for research on new medicines. The neoliberal model of globalisation has induced a decline in state power, both with regard to establishing national research for health priorities and to influencing the (...) development of adequate ethical guidelines to protect human beings that participate in multinational research. In this respect, poor and low-income countries, which lack sustainable control and review systems to deal with the ethical and methodological challenges of complex studies conducted by researchers from affluent countries and funded by large multinational pharmaceutical companies, are particularly vulnerable. The aim of the present paper is to explore critically some of the actual and possible ethical pitfalls of globalisation of clinical research and propose mechanisms for turning transnational clinical research into a more cooperative and fairer enterprise. Confidentiality is a central bioethical principle governing the provider–patient relationship. Dating back to Hippocrates, new laws have interpreted it for the age of precision medicine and electronic medical records. This is where the discussion of privacy and technology often ends in the scientific health literature when Internet-related technologies have made privacy a much more complex challenge with broad psychological and clinical implications. Beyond the recognised moral duty to protect patients’ health information, clinicians should now advocate a basic right to privacy (...) as a means to safeguard psychological health. The article reviews empirical research into the functions of privacy, the implications for psychological development and the resigned sentiment taking hold regarding the ability to control personal data. The article concludes with a call for legislative, educational and research steps to readjust the equilibrium between the individual and ‘Big Data’. (shrink)

The academic literature in research ethics has been marked in the past decade by a much broader focus on the need for the protection of developing communities subjected to international clinical trials. Because of the proximity of the revision of the Declaration of Helsinki, completed in October 2008, most papers have addressed the issue of a double standard of care following the use of placebo. However, other no less important issues, such as interactions between the lifestyles structures of low-income communities (...) and the efficiency of risk-minimising procedures also deserve attention. The purpose of this paper is to discuss forms of uncertainty involved in clinical trials in poor and low-income countries that are not addressed by conventional methods of risk assessment. Furthermore, the increase in size of risks that are identified by conventional assessment methods will be addressed. Besides, the difficulty in properly applying risk-minimising procedures will be discussed. Finally, this paper proposes the involvement of research ethics committees in the risk evaluation process and the establishment of national ethics evaluation systems. (shrink)

Tyler Burge is famous for defending primitivist naturalism about mental representations, according to which mental representations are primitive natural states. Primitivist naturalism contrasts with semantic reductionism, according to which mental representations are reducible to more fundamental natural states. Burge developed the most compelling and influential attack on semantic reductionism from a primitivist naturalist point of view. My goal in this paper is to defend semantic reduc- tionism from Burge’s attack. I assess and refute his objection to the motivations for semantic (...) reductionism and his objection to the semantic reductionist theory that he takes to be the most promising one, namely, teleosemantics. I argue that in spite of the ingenuity of Burge’s objections, they fail to show that there is no motivation for taking representational states as reducible to more fundamental natural states or that the core thesis of teleosemantics is flawed. Inicialmente, considera-se como as possibilidades de relações entre a psicanálise e a filosofia de Peirce têm sido objeto de trabalhos de filósofos e psicanalistas. São destacadas as noções de Winnicott sobre os objetos e fenômenos transicionais e o brincar, como uma área de particular interesse para o estudo dessas relações, dadas suas ligações com a filosofia de Peirce. É, então, indicado como Winnicott faz referência às relações da simbolização com os objetos e fenômenos transicionais e indica como um “full understanding” (...) dos processos de simbolização podem contribuir para o conhecimento desses fenômenos. Considerando essa manifestação de Winnicott, é examinado de que modo a semiótica de Peirce contém elementos que auxiliam o entendimento do que é chamado de “simbolização”, por Winnicott. Assim,, também permitindo a discriminação dos aspectos semióticos presentes nos objetos e fenômenos transicionais e no brincar, permitindo, assim, uma ampliação de algumas dimensões da significação das concepções de Winnicott. As questões referidas são examinadas amplamente nos casos clínicos de “Edmund” e “Diana”, citados por Winnicott e em outras duas situações clínicas apresentadas pelo autor. (shrink)

The number of national bioethics commissions has burgeoned since the establishment of the first one in 1983. They provide an arena in which stakeholders with widely differing moral views can discuss, interact and negotiate about controversial matters. The establishment of the Brazilian committee is used as an example of how such bodies can be introduced. If such councils are to be implemented effectively and regarded as legitimate, the society as a whole should be included in the construction of the proposal (...) and represented on the council, the council should have the benefit of specialist advice when that is needed, and the council should be linked to the elected government in an official advisory capacity. The article describes long process of planning and consultation to establish Brazil's National Bioethics Council and of eventually defining its task as advising the president on matters relating to bioethics. Supervenience is one of the 'hot discoveries' of analytic philosophy, and this collection of essays on the topic represents an examination of it and its application to major areas of philosophy. The interest in supervenience has much to do with the flexibility of the concept. To say that x supervenes on y indicates a degree of dependence without committing one to the view that x can be reduced to y. Thus supervenience is a relationship that has the potential of replacing (...) the traditional notion of dependence, while performing at least part of the function reductive relationships were supposed to fulfil. Desde la tradición de los estudios de gubernamentalidad y sus resonancias en las sociologías políticas y de la educación, en este artículo se recuperan los debates vigentes en torno a las nociones de gubernamentalidad, gobierno y biopolítica desarrolladas en la vasta obra de Michel Foucault y sus continuadoras/es para enlazarlas con las discusiones sobre el objeto de estudio y el campo teórico de la política educativa, enfatizando el potencial analítico que ofrece su grilla de inteligibilidad para los fenómenos educativos propios (...) de las sociedades posdisciplinares. En esa dirección, este trabajo esboza una serie de propuestas epistemológicas, teóricas y metodológicas producidas a partir de una investigación empírica recientemente finalizada (Aguirre, 2022), de corte descriptivo y de carácter cualitativo. En vistas a estos propósitos, se producen diálogos interdisciplinares a la vez que se tensionan y discuten algunos supuestos clásicos y tradicionales de la política educativa, para incitar una renovación analítica que posibilite el estudio de las relaciones sociales de autogobierno y gobierno de las/os otras/os en tanto articulaciones históricas entre el saber, el poder y la subjetividad. Finalmente, se exponen algunos indicios y propuestas para el abordaje sociológico de los fenómenos educativos contemporáneos, que convocan y promueven la necesidad de producir ciertos desplazamientos epistemológicos en el campo teórico de la política educativa, así como invitan a la reconfiguración de sus objetos de estudio en el marco de la adscripción a un enfoque posestructuralista. This article contributes to the literature on national varieties of socially responsible investment (SRI) by demonstrating how Scandinavian SRI developed from the 60s and onwards. Combining findings on Scandinavian SRI with insights from previous research and institutional theory, the article accounts for the role of changes in societal values and norms, the mechanisms by which SRI practices spread, and how investors adopt and transform practices to suit their surrounding institutional contexts. Especially, the article draws attention to how different categories of (...) investors act as institutional entrepreneurs during specific historical periods, and how these roles come to shift as institutional rule systems of varying societal levels change. Thus, the insights gained are useful in the future research agenda concerned with advancing knowledge on idiosyncrasies and commonalities of national SRI manifestations, and to understand the reasons underlying such characteristics.
